Please take note of the following:
Event: Annual Health Visit by School Health Service Team
Date: 2 Jul to 21 Jul (excluding Sat & Sun)
Time: During school curriculum time
Venue: Library

The following items have been distributed to the pupils through the Form Teachers:

1. Letter to parents regarding the visit
2. Information Booklet on "School Health Service: Health Check andImmunisation for P1 Pupils"

• Medical information form (found inside the Information Booklet,pages 2 & 3)
• Consent form for immunisation (found inside the InformationBooklet, pages 4 & 5)

Do submit you child's health booklet, Medical Form and Consent form to your form teachers as requested. Thank you.
